Hemorrhagic ovarian cyst

Presentation A young woman presents with acute onset of left-sided pelvic pain of 2 days duration. Her last menstrual period was 15 days ago. An ultrasound of the pelvis was performed. 
 
 
 
Caption: Transverse transvaginal view of the left ovary
Description: There is a large cyst seen in the left ovary. The cyst shows a reticular pattern of internal echoes. Fine septations are also seen within the cyst. This is highly suggestive of a hemorrhagic cyst.

Description: The cyst is devoid of any color.
 
 
 
Caption: Transverse power Doppler view of left ovarian cyst
Description: As expected, the cyst does not show any vascularity on the power Doppler study.
 
 
 
Caption: Follow up ultrasound of the left ovarian cyst
Description: A repeat ultrasound scan was performed a month later. A decrease in the cyst size is noted. The echogenic blood in the cyst shows evidence of clot retraction.
