Pelvic carcinomatosis due to bilateral adenocarcinoma of the ovaries

Presentation This is an ultrasound of the pelvis in a post-menopausal woman with left lower quadrant pain.
 
 
 
Caption: Sagittal scan of the uterus
Description: The uterus appears normal in size. The endometrial stripe is not very well delineated; however no obvious abnormality is noted.
 
 
 
Caption: Transverse scan of the adnexae
Description: Both the adnexae show the presence of solid appearing heterogeneous masses that appear to be contiguous across the midline. There are bright reflectors seen within the masses which most likely represent calcifications. The ovaries could not be identified separately from these masses.
 
 
 
Caption: Transverse color Doppler scan of the right adnexal mass
Description: The solid component of the complex right adnexal mass exhibits internal vascularity.
 
 
 
Caption: Transverse spectral and color Doppler scan of the left adnexal mass
Description: The left adnexal mass is also very vascular and has a low-resistance arterial waveform that is recorded from one of the vessels within the mass.
 
 
 
Caption: Axial CT scan of the pelvis
Description: A large soft tissue mass was noted in the pelvis extending into the omentum superiorly. The pelvic viscera could not be clearly delineated from this mass.

Final Diagnosis Pelvic carcinomatosis due to bilateral adenocarcinoma of the ovaries
 
Follow Up A biopsy of the omental mass was performed, which was consistent with the diagnosis of ovarian papillary serous carcinoma.
