Caption: Sagittal image of the left adnexa
Description: There is a complex left adnexal mass noted which is compatible with a dilated fallopian tube filled with blood. The echo-free tubular structure represents an iliac vessel.

Caption: Magnified sagittal view of the left adnexal mass
Description: The dilated blood filled left fallopian tube is seen in this image.

Caption: Transverse view of the left adnexal mass
Description: The blood filled fallopian tube is again appreciated in this image. A relatively hypoechoic rounded area is seen within the tube, which possibly is the poorly formed gestational sac. There is no evidence of free fluid in the pelvis.

Caption: Color Doppler of the left adnexal mass
Description: No flow is appreciated within the fallopian tube or in the suggested gestational sac. Note the color in the iliac vessels.

Differential Diagnosis Left unruptured tubal ectopic gestation with hematosalpinx

Final Diagnosis Left unruptured tubal ectopic gestation with hematosalpinx

Follow Up A left salpingectomy was performed and the diagnosis of unruptured tubal ectopic gestation was confirmed.
